We describe a non-obese patient with debilitating symptoms on haemodialysis, with a past history of IIH and previous LP shunt. LP shunt was revised to include a one-way valve but still symptoms were disabling. ICP monitoring was undertaken by our neurosurgical team on the outpatient dialysis unit to further investigate the on-going headaches.Previous cases have been reported of ICP monitoring while patients receive RRT but we believe this is the first to be reported wherein the patient had an LP shunt with a one-way valve leading to novel postural management of headache/DDS. During a period of successful transplantation, headaches completely resolved and recurred as soon as HD was recommenced. We have concluded that our patient did indeed suffer raised ICP during HD as part of a Disequilibrium phenomenon; clearly this is an interesting discussion point.
